In the event of any injuries this is to certify that I, parent or guardian of camper, hereby grant permission to staff of ReJoyce Academy to obtain care from any licensed physician, hospital or medical clinic for the player named herein at such time as either parent or guardian will be contacted in person or by telephone.
**This authorization shall also include all activities and we do hereby waiver, release, absolve, indemnify ReJoyce Academy and staff, Seattle Public Schools, sponsors, the organizers, supervisors, and participants from any claim arising out of an injury to the player or accidents that may occur during the camp.
Registrants to the education and basketball fitness academy camp understand that publicity photographs may be taken during the event and their image/photographic likeness may appear in photographs taken during the camp. By registering for the Rejoyce Academy Health & Safety Hoops Camp, you hereby give permission that photos containing your image/likeness may be used for publicity or general information purposes including publication on the ReJoyce Academy, Department of Commerce, Community Navigator's, and Tukwila School District's website and social media or within other publicity which may be seen by the general public. The ReJoyce Academy will not seek any further permission nor provide any notification before using such photos.
